NO, you can just show up.

Yes, because Italy consulate in LA is very strict on insurance. They have three major requirements:
1. coverage is above $30,000 EURO ($50,000)
2. cover your whole trip in Europe
3. cover all the medical expenses, such as hospitalization, prescription...even if you die in Europe, the expense on shipping your remain to US or China need to be covered as well. Your school insurance will most probably only cover emergency overseas.

I did not get visa when I visited them because of the insurance and hotel reservation (only lack of Paris and Florence). Then I sent by mail and it took one month to get it finally. My whole trip is 23 days and they only give me 24 day visa.
I will never apply schengen visa from Italy consulate any more if I have other choice.
