Let me elaborate on this.
1) You can buy prepaid SIM cards at millions of places such as book stores, gas stations, etc. The card of course comes with a number. You can always recharge the card buy calling customer service, online, or buy another recharge card.
2) Making international calls from your cell phone will be very expensive. So you have to get an IP card and dial the 800 number on the card with access code. If you have used IP cards in China then it works exactly the same way.
3) Make sure your cellphone can use 850MHz or 1900MHz bands.
It will be better if you have a world phone (quad band phone). Otherwise you will have to find out what company operates on which band and then get a SIM card you can use.

手机应该无所谓吧， 卡的话满大街都是， 随便买个号冲值就用了，
另外美国的手机的好处是打任何美国/加拿大的电话都是一个价钱, 没有国内长途一说, 所以即使你所在城市没有Local Access Number也无所谓

有的电话打加拿大是要额外收钱的...我用的tmobile的family plan,打加拿大就多收了...
